• Co-payments, co-insurance, and deductibles are due at the time of service. We do our best to estimate the cost based on information provided by your insurance company.
• All charges not covered by insurance will be the patient's responsibility.

Patients with HMOs must select one of our Providers as their PCP BEFORE their appointment.

• We out-source billing to PGM. If you would like to discuss your bill with PGM, please call 1-888-336-8283 or call our office for more information.
• Questions concerning laboratory or radiology must be directed to the company performing the service or testing. For questions concerning a Labcorp or Quest bill,  please call the phone number located on the bill you received.
• Delinquent accounts: An account becomes delinquent after 90 days overdue. If a payment is not made, the account will be turned over to a third-party collection agency. All balances must be paid in full before any further services are rendered by our office.
